All items are obtainable through gameplay and the extras are for the ONLINE mode ONLY. Single player remains unchanged. TLDR; the premium just gives players a head start in the online mode.
You need Xbox Live Gold to do any online multiplayer with any game. What they mean by free access to GTA Online is you don't have to pay for online as if it's another game itself.
