When you buy the disc version, the campaign is included, although you will have to install the game from the disc. It is about 50 gb.
To my knowledge, you need an internet connection for ALL Xbox One games for the 3-6+ hour long download of the necessary updates before you can play any game. As far as gameplay goes, you'll need internet for about 85% of the game's content as it's main purpose is an online shooter. The story is very short and the arcade mode leaves much to be desired.
If you have the game downloaded and the disk in then you set your account as your home Xbox in settings everyone on that Xbox will have the game.

Thanks for your question! When you choose the 'new' version you are buying a physical disk which will be shipped or picked up in store. For the digital/download version you will receive instructions and a code to download the game directly with no disk. The included game/content is the same.
